Roles of CLASS Providers

Case Management Agency

This agency is contracted with HHS to assist with accessing non-waiver funds so that CLASS funds are only used as a last resort.

Your Case Manager is responsible for:

  • coordinating team meeting
  • draft budgets for CLASS services
  • maintaining monthly contact
  • completing paperwork for requests from CLASS
  • monitoring service delivery

Direct Service Agency

This agency is a licensed Home Health Agency as well as being contracted with HHS to provide staffing of home health services.

The DSA must:

  • send a nurse for an annual assessment to qualify for CLASS; and also send a nurse after any hospitalization.
  • participate in team meeting
  • agree to all budgets for CLASS services

If you request these Services, the DSA is responsible for staffing or purchasing them:

  • Dental
  • Adaptive Aids
  • Minor Home Modifications
  • Pre-Vocational Habilitation (aka Day Hab)
  • Therapies (Music, Massage, Aquatic, Recreational, Therapeutic Horseback, Behavioral Support, Hippotherapy, Auditory Enhancement/Auditory Integration Training)
  • Dietary Services

Finally, there are services that management of can be determine by your choice of Service Delivery Option. Services assigned to the DSA will be listed on your 1st budget page. Services assigned to CDS will be listed on your 2nd budget page.

  • CFC PAS/Hab (attendant services)
  • Occupational therapy
  • Physical therapy
  • Speech therapy
  • Respite
  • LVN or RN nursing
  • Supported Employment
  • Transportation – Habilitation
  • Employment Assistance
  • Cognitive Rehabilitation Therapy

Financial Management Service Agency

This agency is your partner if you select the Consumer-Directed Service Delivery Option.

They will bill HHSC for the services delivered through CLASS, and pay out to your employees and for payroll taxes.
