CLASS Yearly Cycle

Month 1: New budget starts.

Month 3: Case Manager comes to home for 1st Quarterly visit.

Month 6: Case Manager comes to home for 2nd Quarterly visit.

Month 9: Case Manager comes to home for 3rd Quarterly visit.

Month 9-10: DSA nurse will visit for annual Nursing Assessment.

Month 10: Service Planning Team meeting (SPT) with Family, Case Manager, and DSA representative will make the Renewal budget for the next year.
