Home modifications are those services that assess the need to arrange for and provide modifications and/or improvements to the individual’s living quarters to allow for community living and ensure safety, security and accessibility. Minor home modifications (MHM) do not include major home renovation, remodeling or construction of additional rooms.
CLASS Minor Home Modifications are limited to $10,000 total throughout your life.
We have a list of categories that encompasses what we can request as a Minor Home Modifications. Our request needs to fit somewhere in these categories. This is CLASS Provider Manual Appendix II.
Common Minor Home Modifications Funded for My Clients
- Door widening (category 2. Specialized Accessibility/Safety Adaptations/Additions – C. door widening)
- Ramps (category 2. Specialized Accessibility/Safety Adaptations/Additions – C. ramps (constructed to provide access into and within the home)
- Roll-in showers (category 1. Home Modifications – D. roll-in showers)
- Grab bars (category 2. Specialized Accessibility/Safety Adaptations/Additions – G. grab bars and handrails)
Steps to request CLASS Minor Home Modification funds:
- Fill out Form 3660 Part A, and give both Form 3660 to your Case Manager to start the CLASS MHM request.
- Your Case Manager will complete Form 3660 Part B with:
- Documenting that this item falls into a category in Appendix II of the CLASS Provider Manual.
- and forward it to your CLASS DSA.
- Your DSAs must arrange for the appropriate professional listed next to the item in Appendix II to complete Form 3660 Part C.
- If you do not already have access to the appropriate professional to complete Form 3660 Part C, funds for an evaluation can be added to your CLASS Budget.
- If requested items estimated cost is over $1000, funds for appropriate professional to complete Form 3849-A Specifications can be added to your CLASS Budget.
- If either of these additions are needed, these additional steps will occur:
- DSA will request funds for professional eval and/or specs be added to your budget.
- Your Case Manager will complete the budget change, send it to you to sign.
- Return the signed budget to your case manager.
- Your Case Manager will submit the budget and it will be effective in 30 days.
- Once the CLASS Budget is approved, your DSA will contract with professionals to complete the needed forms.
- For purchases over $1000,
- Your DSA will send Form 3849-A for you to sign to signify agreement with the specifications.
- Return the signed Form 3849-A to your DSA.
- Your DSA must obtain comparable bids for the requested MHM from three vendors.
- Your DSA will complete Form 3660 Part D, bid(s), (and Form 3849-A if needed) and send a request to your Case Manager to add funds to CLASS budget.
- Your Case Manager will complete the budget change and send it to you to sign.
- Return the signed budget to your case manager.
- Your Case Manager will submit the budget and it will be effective in 30 days.
