- “Related Condition”: IQ < 75
- Function ability: Level of Care I for ICF-MR

- Personal care component: 4 options, included 2 options in home & 2 options in facility with 4 person maximum per house…

- {Home}Foster/Companion Care: Parents/relative paid daily rate to provide assistance to ADULT individual living with them
- {Facility}Residential Support Services: Group home paid daily rate to provide 24-hour awake staff
- {Facility}Supervised Living: Group home paid daily rate to provide 24-hour staff (who can sleep at night)
- Social work: Assessment and direct therapeutic intervention. Training and consultation with family/provider.

I need more information on HCS group home services for two disabled individuals at my church. They are IDD and semi independent with supports and services but would have a better quality of life in a group home setting. They cannot pay the full freight cost of group home living themselves, and would need the financial support that Medicaid offers.
